Job Summary

We are seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team at Envision Home Health & Hospice. As a LPN, you will provide high-quality patient care to individuals in their homes, assisted living facilities, and skilled nursing facilities in the Pierce and Thurston County, WA area.

Responsibilities
  • Conduct thorough assessments of patients to identify their needs and develop personalized care plans.
  • Provide one-to-one care to patients, including administering medications, wound care, and other treatments as prescribed by physicians.
  • Collaborate with healthcare teams to ensure seamless care transitions and effective communication.
  • Document patient progress and maintain accurate records.
Requirements
  • Graduate of an accredited or state-approved nursing school.
  • Licensed as a LPN in the State of Washington.
  • At least 1 year of experience in an acute care setting, with home care or hospice experience preferred.
