Key Responsibilities:
- Collaborate with nurse care managers, social workers, pharmacists, mental health case managers, and other team members to provide comprehensive care to older adults.
- Prescribe medication and provide medical care to patients.
- Participate in interdisciplinary team meetings to discuss patient care and develop care plans.
Requirements:
- Board-certified/board-eligible Family Medicine or Internal Medicine trained physician, or completion of Geriatric Medicine fellowship.
- Interest in obtaining MAT Waiver for prescribing Suboxone.
- Collaborative and team-oriented approach to care.
