Job Title: Clinical Manager RN

We are seeking a highly skilled and experienced Clinical Manager RN to join our team at Affirmed Home Care. As a key member of our clinical leadership team, you will be responsible for developing and implementing patient-specific care plans, ensuring the highest quality of care for our clients.

Key Responsibilities:

  • Develop and maintain patient-specific care plans
  • Ensure the quality of patient care
  • Supervise and support the home care team
  • Strive to improve the lives and sustainability of our clients by supporting their autonomy in the home environment

Requirements:

  • Current RN license in good standing
  • BSN preferred
  • Minimum 2 years recent clinical experience within a Home Care setting
  • Strong assessment and organizational skills
  • Prior supervisory experience with field staff
  • Demonstrated successful track record of working independently
