Job Title: MDS Coordinator

Job Summary:

We are seeking a highly skilled MDS Coordinator to join our team at CommonSpirit Health. As a key member of our interdisciplinary team, you will be responsible for coordinating the MDS/Resident Assessment Instrument and assisting in the ongoing education of those preparing the MDS.

Responsibilities:

* Coordinate the MDS/Resident Assessment Instrument and assist in the ongoing education of those preparing the MDS
* Responsible for the electronic transmission of the MDS/Resident Assessment data to the State Department of Health
* Demonstrate knowledge to complete the MDS/Resident Assessment Instrument
* Facilitate communication between the interdisciplinary team
* Assist in the assessment of the resident, in the planning of care, and in the evaluation of the plan of care, according to the policies and procedures while maintaining standards of the State Nurse Practice Act

Essential Duties and Responsibilities:

* Schedule and complete the MDS and RAP's according to the State and Federal guidelines
* Input data into the computer system
* Assist and train other members of the interdisciplinary team to complete their portion of the MDS
* Complete resident interviews as needed for completion of the MDS
* Transmit MDS data to the State Department of Health

Requirements:

* Nursing degree from an approved school of nursing
* Licensure: Licensed to practice registered nursing in the state of ND or in a compact state
* Experience: Basic MDS training preferred
