MDS Coordinator

Catholic Health Initiatives is seeking a highly skilled MDS Coordinator to join our team. As a key member of our interdisciplinary team, you will be responsible for coordinating the MDS/Resident Assessment Instrument and ensuring the accurate and timely completion of resident assessments.
Key Responsibilities:
- Coordinate the MDS/Resident Assessment Instrument and ensure accurate and timely completion
- Assist in the ongoing education of staff preparing the MDS
- Facilitate communication between the interdisciplinary team
- Assist in the assessment of residents, planning of care, and evaluation of care plans
- Screen for Skilled Nursing Facility admits
Requirements:
- Nursing degree from an approved school of nursing
- Licensed to practice registered nursing in the state of ND or in a compact state
- Basic MDS training preferred
