Job Summary

We are seeking a highly skilled MDS Coordinator to join our team at CommonSpirit Health. As a key member of our interdisciplinary team, you will be responsible for coordinating the MDS/Resident Assessment Instrument and ensuring the accurate and timely transmission of data to the State Department of Health.

Key Responsibilities:

  • Coordinate the MDS/Resident Assessment Instrument and assist in the ongoing education of team members
  • Ensure the accurate and timely transmission of MDS data to the State Department of Health
  • Facilitate communication between the interdisciplinary team and assist in the assessment of residents, planning of care, and evaluation of care plans
  • Assist in screening for Skilled Nursing Facility admits and participate in Fall and Psychosocial committees

Requirements:

  • Nursing degree from an approved school of nursing
  • Licensed to practice registered nursing in the state of ND or in a compact state
  • Basic MDS training preferred
