Palliative Care RN Hybrid Job Description

We are seeking a highly skilled Palliative Care RN Hybrid to join our team at UnitedHealth Group. As a Palliative Care RN Hybrid, you will play a critical role in providing high-quality, patient-centered care to individuals with serious illnesses.

Key Responsibilities:
  • Participate in and/or lead iterative goals of care conversations with patients in various stages of serious illness, using advanced clinical communication and a whole-person approach.
  • Create informative and clear documentation, particularly for patients with high complexity in need of contingency planning.
  • Proactively partner and collaborate with TEC colleagues across multiple disciplines and settings to create integrative, robust care plans.
  • Conduct visits with patients and families in the clinic, telephonically, at in-home visits, at hospitals, in skilled nursing facilities, in adult family homes, and other locations as appropriate.
  • Grow provider and staff competency in advanced practice communication skills, serious illness management, and palliative care approach and philosophy of care.
Requirements:
  • Graduation from an accredited school of nursing.
  • Basic Life Support for Healthcare providers (AHA) or CPR/AED for the Professional Rescuer (American Red Cross).
  • Washington State Registered Nurse license.
  • 3+ years of experience in a clinical setting.
  • 2+ years of experience engaging and assisting patients approaching end of life - preferably in inpatient (ED, ICU, oncology), palliative, or outpatient geriatric context.
  • Washington State driver's license and vehicle for work-related travel.
Preferred Qualifications:
  • Experience with complex patients and interdisciplinary teams.
  • Some experience in adult education.
  • Knowledge of palliative care best practices.
  • Knowledge of medical terminology and disease courses.
  • Knowledgeable about community resources appropriate to needs of patients/families.
  • Knowledge of culture and values in the patient population and local community.
