Care Management Associate

Vaya Health is seeking a skilled Care Management Extender to support our care management team in delivering high-quality, patient-centered care. As a Care Management Extender, you will work closely with care managers to provide outreach, support, and education to our members, helping them navigate the healthcare system and achieve their health goals.
Key Responsibilities- Perform general outreach, engagement, and follow-up with members to ensure they receive necessary support and services.
- Coordinate services and appointments, including arranging transportation and providing reminders.
- Engage in health promotion activities, such as providing education on chronic conditions, teaching self-management skills, and sharing resources.
- Share information with the care team on member circumstances and provide referrals and assistance in obtaining community-based resources.
- Participate in case conferences and support care managers in assessing and addressing unmet health-related resource needs.
To be successful in this role, you must have a high school diploma or equivalent and be at least 18 years old. You must also be trained in Tailored Care Management and meet one of the following requirements:
- Be a person with lived experience with an intellectual or developmental disability (I/DD) or traumatic brain injury (TBI) with demonstrated knowledge of and direct personal experience navigating the North Carolina Medicaid delivery system.
- Be a person with lived experience with a behavioral health condition who is a Certified Peer Support Specialist.
- Be a parent or guardian of an individual with an I/DD or TBI or behavioral health condition and have at least two years of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual.
- Have two years of paid experience performing the types of functions described in this role description in addition to one year of paid experience working directly with the Tailored Care Management eligible population.
