We are seeking a dedicated and compassionate Mental Health Advocate to join our team. As a key member of our Transitional Living Program, you will play a vital role in supporting individuals with severe mental illness to achieve their goals and live independently.

Key Responsibilities:

  • Provide case management and support services to program participants, including crisis intervention and counseling.
  • Develop and implement individualized service plans to meet the unique needs of each participant.
  • Collaborate with treatment providers, healthcare professionals, and community agencies to ensure comprehensive care and support.
  • Conduct regular home visits and provide guidance on daily living skills, budgeting, and meal planning.
  • Ensure accurate and timely documentation of participant progress and services provided.
  • Participate in staff meetings and training sessions to enhance skills and knowledge.

Requirements:

  • Bachelor's degree in a human services field or related experience.
  • Valid New York State driver's license and reliable vehicle for travel and transporting clients.
  • Ability to work independently and as part of a team, with minimal supervision.
  • Excellent communication and interpersonal skills, with a warm and non-judgmental approach.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
