Job Summary

We are seeking a highly motivated and compassionate Peer Support Specialist to join our team at WestCare Foundation in Key West, Florida. As a Peer Support Specialist, you will play a vital role in providing support and guidance to individuals receiving medical outpatient mental health services.

Key Responsibilities
  1. Triage phone calls and provide community resource information and supportive counseling.
  2. Conduct screening and assessments, and provide intervention services.
  3. Complete required state and intake paperwork.
  4. Assist in the completion of client admission and orientation procedures and documentation.
  5. Initiate and/or respond to collateral individuals per treatment plan, policies, and procedure.
  6. Communicate a Welcoming Expectation to the client, demonstrate empathic listening and compassion, and establish a hopeful connection in the process of collecting supporting information.
  7. Engage individuals and families in the crisis planning process using evidenced-based practices, including Motivational Interviewing.
  8. Coordinate psychiatric face-to-face or telehealth appointments with prescribers.
  9. Coordinate with doctors and participate in medical treatment planning and reviews; consult with each patient concerning attainment of treatment plan goals and objectives.
  10. Order medications as requested by medical staff/doctors.
  11. Maintain medication inventory.
  12. Record/maintain medication samples and IDP.
  13. Assist clients with medications, prescription assistance plans, or IDP.
  14. Provide after-hours crisis services on a rotation schedule.
  15. Conduct home visits.
  16. Interact with management to gather and provide information.
  17. Provide recovery support through coaching and mentoring services to clients as assigned.
  18. Provide support by identifying potential 12-step groups appropriate for each client and/or member of the family, which is noted in family treatment plans.
  19. May attend 12-step meetings with clients.
  20. Act as an appropriate role model for clients and act as a client advocate.
  21. Respond to clients' questions in a timely manner.
  22. Participate in multidisciplinary staff meetings.
  23. Complete all documentation (including signature when necessary), on client status and behavior, per policies and procedures, and individual treatment plans.
  24. Complete daily Service Activity Log as requested.
  25. Work effectively/closely with Case Managers to assist in the coordination of client services.
  26. Assist clients in making appropriate transportation arrangements as needed.
  27. Contact assigned clients on a weekly basis if absent or de-compensating.
  28. Make referrals for services as needed.
  29. Investigate and use factual data for problem-solving client and/or staff-related conflicts.
  30. Back up front office/reception. Assist with scheduling appointments, super bills/payments, answering phones.
  31. Complete and comply with Request for records.
  32. Responsible for maintaining the patient's chart in compliance with State, CARF, Medicaid, and other rules and standards.
  33. Attend and participate in training activities, staff meetings, and supervisory sessions.
Qualifications
  • Education: Minimum High School Diploma.
  • Experience: Training and/or experience in Crisis Intervention preferred. Familiarity with Monroe County. Minimum of 1 year recovery from addiction and actively involved in a recovery program. Required one year Medical Office experience, preferred six months Mental Health/Substance Abuse background. Ability to meet requirements of Monroe County Sheriff's Office background check. CPR Certification, First Aid Certification, and an annual Tuberculosis test. Ability to provide an Affidavit of Good Moral Character. Clearance in Pre-Employment Drug Testing and Criminal Records Checks, as required.

During your tenure with WestCare, there may be new requirements, including, but not limited to, vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur, Human Resources or appropriate personnel will inform you.
