Med Tech Assistant

Discovery Commons San Pablo is seeking a skilled Care Giver/Medication Technician to provide exceptional care and support to our residents. As a key member of our team, you will play a vital role in ensuring the health, safety, and well-being of our residents.
Responsibilities- Provide hands-on care and physical support to residents, maintaining a safe and comfortable living environment.
- Observe, report, and document changes in resident conditions, including skin, behavior, alertness, weight, dietary, and participation in activities.
- Respond promptly to security system alerts and resident call bells.
- Notify supervisors and/or Health Care Coordinators of increased care needs or concerns.
- Assist with continence management, dispose of products properly, and maintain cleanliness of resident suites and community restrooms.
- Serve and collect food trays, assist as needed, and report meal refusals or loss of appetite.
- Maintain resident rooms and work areas in a clean and organized state.
- Practice good standard care precautions, including cleanliness, hygiene, and health.
- Help residents maintain independence, promote dignity, and physical safety.
- Assist with activities of daily living, such as bathing, dressing, toileting, grooming, ambulation, transferring, and eating.
- Engage residents in life skills and other life enrichment activities.
- Greet and assist internal and external customers, guests, family members, residents, vendors, and team members.
- Perform resident laundry as assigned and needed.
- Review service plans to learn pertinent information about residents.
- Assist with medication administration, observing the Six Rights of Medication Pass.
- Document and initial medication records, including refusals or missed doses.
- Maintain confidentiality of resident information, including medication.
- Report concerns or issues related to medication administration to Nurses and/or Health Care Coordinators.
- Restock medication carts after each pass.
- Assist with medication checks, regardless of packaging systems.
- Count narcotics with another Medication Care Manager or Nurse each shift.
- Maintain and clean the Medication Room, med charts, treatment carts, and expired medications.
- Follow re-fill processes for medications.
- Participate in developing and updating Service Plans.
- Take and record vital signs, including temperature, blood pressure, weight, pulse, and respiration rates.
- Report occupational exposures to blood, body fluids, infectious materials, sharp sticks, and hazardous chemicals immediately.
- Certified Nurse's Aid certification preferred.
- High School diploma/GED.
- Must be 18 years of age.
- CPR Certification preferred.
- First Aid Certification preferred.
- Previous experience working with seniors preferred.
