Medical Social Worker Specialist

As a Per Diem Medical Social Worker at Andwell Health Partners, you will play a vital role in enhancing the quality of life for our patients by providing compassionate and innovative healthcare services in their homes and communities. This position requires a Master's degree from a school of social work accredited by the council on social work education, Maine Board of Social Work Licensure: LCSW Required, and one or more years of experience as a master's level social worker in a healthcare setting.
Key Responsibilities
Assessing and identifying clients' holistic health care needs and concerns as part of a family and community.
- Developing plans of care under physician orders with realistic and measurable outcomes in collaboration with clients, families, and healthcare teams.
- Implementing patient care plan activities based on treatment goals towards independent living for patients and families.
This Per Diem LCSW position supports Palliative Care, Hospice, and Home Health. You will also serve as a resource for staff education & training and participate in case conferences as needed.
With your expertise, you will help patients achieve better health outcomes, improve their quality of life, and receive the necessary support to maintain independence.
