We are seeking a Licensed Practical Nurse (LPN) to join our team at Providence Home Health in Woodburn, OR. As an LPN, you will work alongside an interdisciplinary team to provide high-quality care to patients in their homes. This is a full-time, day shift position with a competitive salary and benefits package.
Responsibilities- Provide whole-person care to patients in their homes, working collaboratively with the interdisciplinary team to develop and implement individualized care plans.
- Conduct thorough assessments of patients' physical, emotional, and social needs, and develop plans to address these needs.
- Administer medications and treatments as prescribed by physicians, and monitor patients' responses to these interventions.
- Collaborate with patients, families, and caregivers to educate them on disease management, self-care, and health promotion.
- Participate in quality improvement initiatives to ensure high-quality care and patient satisfaction.
- Current Oregon LPN licensure
- 2 years of patient care experience in a skilled nursing facility or medical clinic
- Ability to work independently and as part of a team
- Excellent communication and interpersonal skills
- Ability to work in a fast-paced environment and prioritize tasks effectively
- Experience in Acute Care, Home Health, or Public Health nursing
