Job Summary

We are seeking a Licensed Practical Nurse (LPN) to join our team at Providence Home Health in Woodburn, OR. As an LPN, you will work alongside an interdisciplinary team to provide whole-person quality care to patients in need.

Key Responsibilities
  • Provide patient-centered care to patients in their homes, focusing on their physical, emotional, and spiritual needs.
  • Collaborate with the healthcare team to develop and implement individualized care plans.
  • Conduct thorough assessments, identify patient needs, and develop effective care strategies.
  • Administer medications, perform treatments, and provide wound care as needed.
  • Document patient progress, communicate with the healthcare team, and maintain accurate records.
Requirements
  • Current Oregon LPN licensure.
  • 2 years of patient care experience in a skilled nursing facility or medical clinic.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Ability to lift, push, and pull up to 25 pounds.
Preferred Qualifications
  • Experience in Acute Care, Home Health, or Public Health nursing.
