Job Summary:

As a Licensed Practical Nurse with Providence Home Health, you will play a vital role in delivering high-quality patient care in the comfort of their own homes. Our interdisciplinary team works together to provide whole-person care, empowering nurses to create practice standards and expectations. With our shared governance model, you will have the autonomy to exercise independence and flexibility in your career.

Responsibilities:

Provide skilled nursing care to patients in their homes, focusing on their unique needs and goals.

Collaborate with the healthcare team to develop and implement individualized care plans.

Conduct thorough assessments, identify areas for improvement, and implement evidence-based interventions.

Educate patients and their families on disease management, medication adherence, and self-care techniques.

Document patient progress, identifying areas for improvement and implementing changes as needed.

Requirements:

Current Oregon Licensed Practical Nurse (LPN) licensure.

National Provider BLS - American Heart Association certification within 30 days of hire.

2 years of patient care experience in a skilled nursing facility or medical clinic.

Preferred qualifications: experience in Acute Care, Home Health, or Public Health nursing.
