About the Role

We are seeking a highly skilled Geriatric Physical Therapist to join our interdisciplinary team at Mother Joseph Care Center in Olympia, Washington.

As a Geriatric Physical Therapist, you will be responsible for developing and implementing individualized therapy plans to help patients achieve their optimal level of independence and function. You will work closely with an interdisciplinary team to coordinate care and ensure seamless transitions between therapies.

Responsibilities
  • Conduct comprehensive assessments and evaluations to identify patient goals and develop personalized treatment plans.
  • Implement evidence-based physical therapy interventions to improve patient outcomes and quality of life.
  • Collaborate with healthcare teams to ensure comprehensive care and effective communication.
  • Document patient progress and maintain accurate records of treatment plans and outcomes.
Requirements
  • Bachelor's Degree in Physical Therapy from an accredited program.
  • Current licensure as a Physical Therapist in the state of Washington.
  • National Provider BLS - American Heart Association certification upon hire.
  • Minimum 1 year of experience working with geriatric populations.

Providence offers a competitive salary range of $46.44 - $73.53 per hour, depending on experience. Additional compensation may be available for shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
