We are seeking a skilled and compassionate Registered Nurse to join our team in Manhattan, NY. As a Senior Medical Care Coordinator, you will play a crucial role in ensuring the delivery of high-quality patient care.

Job Summary
  • Supervision of Home Healthcare Professionals: You will oversee the work of home healthcare personnel to ensure that care delivery meets professional and regulatory standards. This includes conducting initial visits, supervisions, and reassessments as needed.
  • Care Coordination: Collaborate with the Director of Nursing and other healthcare providers to develop comprehensive, individualized care plans based on doctor's orders.
  • Patient Assessments: Conduct in-home assessments and reassessments of patients' conditions, reviewing and modifying care plans as necessary. Ensure accurate and timely documentation of assessments.
  • Compliance and Quality Assurance: Ensure compliance with state and federal regulations, accreditation standards, and agency policies. Assist in conducting quality assurance checks, audits, and reviews.

Key Requirements:

  • Bilingual: Spanish language proficiency is required.
  • Proficiency in electronic health record systems and documentation.
  • Able to travel within the agency's service area for patient visits and staff supervision (Manhattan and the Bronx).
  • Flexibility to work weekends and evenings, if required.
