Care Coordinator for Mental Health Services

About the Position
We are seeking a dedicated Care Coordinator to join our team at the Mental Health Association of Nassau County. The successful candidate will provide supportive services to individuals in our Supportive Housing Program, helping them achieve their personal recovery goals and live independently.
Job Summary
The Care Coordinator will be responsible for conducting weekly visits with participants, providing face-to-face contact, and offering supportive services tailored to their needs. This includes daily living skills training, assessment for and linkage to community services, and education on being a responsible tenant.
Key Responsibilities
- Conduct regular home visits to assess participants' needs and progress toward their recovery goals
- Provide crisis management services, including hospital visits, coordination with treatment teams, and post-discharge aftercare
- Assist participants in developing realistic and measurable recovery plans that reflect their cultural background and values
- Collaborate with landlords, treatment teams, and other stakeholders to ensure seamless support for participants
- Conduct fire safety and disaster preparedness drills, and oversee participants' living areas to ensure health and safety standards are met
- Complete reports using Electronic Medical Records, and ensure all required documentation is completed in a timely manner
Requirements
- Bachelor's degree in a human services field, or related experience
- CPR and First Aid certification
- Minimum 2 years of experience working with individuals diagnosed with mental illness
- Valid NYS driver's license and own transportation
- Proficiency in MS Word and basic internet navigation
- Excellent written and oral communication skills, with patience, tact, and a cheerful disposition
Compensation and Benefits
The salary for this position is $60,000-$70,000 per year, depending on experience.
