Job Summary:

We are seeking a Registered Nurse with strong clinical skills and a passion for delivering personalized care. The ideal candidate will be able to work independently and as part of a team to coordinate patient care, develop and implement care plans, and provide education and support to patients and their families.

Responsibilities:

  • Provide skilled nursing care to patients in their homes, including assessments, treatments, and interventions.
  • Develop and implement individualized care plans in collaboration with patients, families, and healthcare teams.
  • Coordinate patient care, communicate effectively with healthcare teams, and ensure seamless transitions between care settings.
  • Educate patients and families on disease management, self-care techniques, and prevention strategies.
  • Maintain accurate and comprehensive records, adhering to regulatory requirements and standards.

Requirements:

  • Current, unrestricted state license as a Registered Nurse.
  • Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
  • One year of experience working under current nursing license; six months of recent experience as a Licensed Nurse in a clinical care setting preferred.
  • CPR certification and continuing education as required by state.

Physical Requirements:

  • Ability to lift 50 pounds, walk, stand, bend, kneel, reach, twist, sit, and climb stairs.
  • Visual and hearing acuity; strong sense of smell and touch.

Work Environment:

  • Must be able to function in a wide variety of environments, including exposure to allergens and other conditions.
  • Possible exposure to blood, bodily fluids, and infectious diseases.

Compensation and Benefits:

Competitive salary range: $27-$32 per hour; benefits package includes medical, dental, vision, and retirement savings options.
