Position Overview
The Department of Corrections is on the lookout for a dedicated and skilled Pharmaceutical Care Specialist to join our team at the Clallam Bay Corrections Center (CBCC). This full-time role is structured around a day shift schedule with weekends off.
CBCC is responsible for the care of 858 male individuals situated on the Olympic Peninsula, offering a unique working environment with a focus on enhancing health outcomes for our patients.
The primary function of the Pharmaceutical Care Specialist is to elevate patient health and wellness through evidence-based pharmaceutical practices. This role is pivotal in delivering clinical services to patients within DOC facilities and aiding their transition back into the community. The key objectives include optimizing therapeutic results, ensuring cost-effective medication management, and providing education to both staff and patients regarding appropriate medication use, thereby promoting best practices in patient-centered care.

The Pharmaceutical Care Specialist will be responsible for managing pharmaceutical resources in line with DOC Pharmacy and Therapeutics (P&T) Committee directives, DOC Health Services Policy, and relevant laws.
Specific duties include:
- Collaborating as a vital member of a multidisciplinary team to deliver evidence-based, patient-focused pharmaceutical care.
- Maintaining expertise in DOC Health Plan services and ensuring safe and effective medication therapy.
- Conducting patient assessments and monitoring to identify and resolve drug-related issues.
- Providing comprehensive medication recommendations and consultations to healthcare professionals.
- Utilizing patient health records for effective communication and documentation of pharmacotherapeutic recommendations.
- Participating in clinical rounds and offering management solutions for medication therapy.
- Leading and participating in medication management programs.
- Delivering patient education and counseling.
Required:
- Minimum of two years of clinical pharmacy experience.
- A valid Washington State Pharmacist License.
- Completion of a pharmacy residency.
- Board of Pharmacy Specialties certification in relevant areas.
- Experience in Hospital, Long Term Care, or Managed Care Pharmacy.
