Job Summary

Matrix Medical Network is seeking a skilled Nurse Practitioner to join our team as a Home Risk Assessment Specialist. This is a PRN/Part Time opportunity that offers flexible scheduling and the chance to make a meaningful impact on the health and well-being of others.

Key Responsibilities
  • Conduct comprehensive health assessments of patients in their homes, identifying potential health risks and developing personalized care plans.

Requirements
  • Master's degree in Nursing or related field.
  • Current RN and NP licensure, with prescription authority.
  • Board certification in a relevant specialty.
  • 1+ year of experience as a Nurse Practitioner.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a team.
