About the RoleWe are seeking a skilled Nurse Practitioner to join our team in Buffalo, Texas. As a Nurse Practitioner with Matrix Medical Network, you will have the opportunity to work with a variety of patients, assessing their health needs and developing personalized care plans.
This is a PRN/Part Time opportunity with flexible scheduling and the potential for up to $100.00 per completed assessment. Our Nurse Practitioners work in a variety of settings, including homes, skilled facilities, and through telehealth visits.
Responsibilities- Conduct comprehensive health assessments, including medical history, diagnosis, and treatment planning
- Develop and implement personalized care plans to meet the unique needs of each patient
- Collaborate with physicians, case managers, and other healthcare professionals to ensure seamless care coordination
- Provide education and support to patients and their families to promote healthy lifestyle choices
- Maintain accurate and detailed records of patient interactions and care plans
To be considered for this opportunity, you must have:
- A Master's Degree in Nursing or a related field
- Current RN and NP licensure in the state of practice, with prescription authority or the ability to obtain
- Board certification by a recognized nursing organization
- A minimum of 1 year of experience as a Nurse Practitioner
- Strong computer skills and familiarity with electronic health records
- Excellent communication and interpersonal skills
