About the Role:
The primary objective of the Rehabilitation Case Coordinator position is to deliver specialized, comprehensive case management for residents. This role is focused on empowering residents to take charge of their rehabilitation and reintegration processes, ultimately aiming to minimize the risk and severity of re-offending.
The Case Coordinator will function within established case management frameworks designed to align with principles of rehabilitation and reintegration.
Key Responsibilities:
The responsibilities encompass all aspects of case management, including:
- Conducting initial assessments and identifying resident needs.
- Developing case plans that incorporate specialized rehabilitation and reintegration services.
- Facilitating connections with the resident's support networks and external service providers.
- Creating a tailored reintegration strategy.
- Staying informed about resident progress and relevant events.
- Motivating residents through various techniques, including evidence-based practices and motivational interviewing.
- Monitoring resident progress against established plans and conducting regular reviews to ensure all necessary interventions are provided.
- Building and maintaining effective networks to support resident reintegration.
- Ensuring that residents' reintegration needs are addressed prior to their transition back to the community.
- Collaborating with relevant agencies to ensure smooth transitions.
- Providing timely support and guidance to program service providers.
- Assisting in the completion of risk and needs assessments to identify priorities and recommend appropriate case management levels.

Qualifications:
To be considered for this role, candidates should possess:
- A Bachelor's degree or equivalent experience in criminal justice
- Experience working with populations affected by chemical dependency or mental health issues
- Counseling certification is preferred
- A minimum of two years of experience in corrections or a related field is preferred
- Strong communication and writing skills
