Job Summary

The RN Care Coordinator is a critical role at Chi, responsible for overseeing the progression of care and discharge planning for identified patients. This position requires a strong clinical background, excellent communication skills, and the ability to work effectively with multiple stakeholders.

Key Responsibilities
  • Completes and documents discharge planning assessments for patients identified by the designated screening process.
  • Develops and implements multidisciplinary discharge plans, engaging other relevant health team members, patients, and post-acute care providers.
  • Collaborates with the healthcare team to ensure timely and smooth transitions to post-acute services based on patient clinical needs.
  • Identifies risk for readmission and implements interventions to mitigate those risks.
  • Responsible for patient education and advocacy, as well as participating in performance improvement teams and programs.
Qualifications
  • Graduate of an accredited school of nursing.
  • Minimum two years of acute hospital clinical experience or a Master's degree in Case Management or Nursing field in lieu of 1 year experience.
  • RN license in the state(s) covered is required.
  • BLS required within 3 months of hiring if located within hospital.
