We are seeking a skilled Clinical Care Specialist to join our team at Christian Living Communities. As a Clinical Care Specialist, you will provide high-quality clinical services to our residents, including skilled nursing, assisted living, and all shifts. You will coordinate care and services based on resident needs and individual preferences, utilizing your education-based expertise to provide a holistic and person-centered model of care.
Key Responsibilities
- Provide daily direction and/or assistance with residents' activities of daily living.
- Become familiar with residents' preferences to personalize their care.
- Assist residents with social, personal, and health care needs as required.
- Ensure knowledge and proper application of safety, infection control, and risk management procedures.
- Make decisions in emergency situations and notify appropriate personnel.
- Communicate regularly with nursing supervisors regarding residents' status and condition.
- Accurately document and promptly report discrepancies to appropriate personnel.
Requirements
- High School Diploma or equivalent.
- Certified Nursing Assistant License-active and in good standing.
- Ability to work well with different teams in various Communities.
- 3-6 months experience working with senior adults preferred.
- Excellent customer service skills.
Working Conditions
- Position requires strenuous physical work, including heavy lifting, pushing, or pulling objects up to 100 pounds occasionally and/or up to 50 pounds frequently.
- Exposure to blood-borne pathogens, body excretions, and communicable diseases possible.
- Frequent sitting, standing, bending, and moving during working hours.
- Frequent interruptions.
- Interactions with residents, family members, and visitors.
