Job Summary:
The Home Care Licensed Practical/Vocational Nurse is a key member of our healthcare team, providing direct nursing care to patients in their homes. As a Visit Nurse, you will work under the direct supervision of a Registered Nurse and/or designated supervisor, delivering high-quality clinical services within the scope of practice.
Responsibilities:
- Provide high-quality clinical services: Deliver nursing care as prescribed by the physician, following all applicable infection control procedures.
- Document patient visits: Record patient visits thoroughly and completely in the electronic medical record, utilizing the agency's electronic visit verification program.
- Participate in care coordination: Actively participate in the agency's care coordination process, including timely reporting of patient needs and changes in condition, attending patient case conferences, and communicating effectively with the patient, family, physician, and other healthcare team members.
- Ensure patient confidentiality: Maintain the confidentiality of patient health information, adhering to agency policy.
Requirements:
- Current LPN licensure: Must have current licensure as a Licensed Practical Nurse in the state of practice.
- Driver's license and vehicle insurance: Must have a current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
- CPR certification: Must have current CPR certification.
