RN Visit Nurse

We are seeking a skilled Registered Nurse to join our team as a Visit Nurse in Home and Community Based Services. As a member of our team, you will provide high-quality clinical services to patients in their homes, working closely with other healthcare professionals to achieve and maintain continuity of patient care.
Key Responsibilities:
- Provide clinical nursing assessments and develop individualized plans of care in accordance with federal and state program requirements.
- Ensure patient eligibility and medical necessity for services, collaborating with other community providers as needed.
- Develop and revise plans of care to meet the needs of patients, initiating revisions as necessary with physician approval.
Requirements:
- Current RN licensure in the state of practice.
- Minimum of one year experience as an RN, with a strong background in home health care.
- Current CPR certification and a reliable vehicle or access to public transportation.
