Primary Care Team Lead

We're a dynamic team of healthcare professionals dedicated to delivering exceptional patient care. As a Primary Care Team Lead, you'll work closely with our experienced nursing staff to coordinate patient care and provide clinical supervision to LPNs and CTA II. Our team is passionate about making a difference in the lives of our patients, and we're looking for like-minded individuals to join us on this journey.
Job Responsibilities
As a Primary Care Team Lead, you'll be responsible for managing daily huddles to review patient charts for missing labs, reports, and/or open orders. You'll work closely with our care team to ensure seamless patient care coordination and provide clinical supervision to LPNs and CTA II. You'll also be responsible for managing in-basket requests, responding to urgent same-day appointments, and documenting progress notes in our electronic health record within 24 hours.
In this role, you'll have the opportunity to develop and enhance your leadership skills, working closely with our experienced nursing staff to deliver high-quality patient care. We're committed to supporting your growth and development, providing ongoing training and education to help you succeed in your role.
The estimated salary for this position is $80,000 - $105,000 per year, based on location and experience.
