We are seeking a highly skilled Nurse Practitioner to join our team at Optum in Durham, NC. As a member of our interdisciplinary team, you will provide home-based medical care to patients in an assigned geography. Responsibilities:- You will practice in accordance with state laws and regulations governing advanced practice nursing or physician assistants.
- You will establish and maintain effective relationships with patients, caregivers, and healthcare professionals.
- You will perform initial comprehensive assessments and develop patient management care plans.
- You will educate patients and their representatives on disease processes and ways to manage them effectively.
- Completion of a Nurse Practitioner accredited program
- Current NP license
- Current state RN license
- Knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems
The estimated salary for this position is $120,000 - $150,000 per year, depending on experience and qualifications. In addition to a competitive salary, we offer a comprehensive benefits package, including student loan repayment and career advancement opportunities.
