Job Summary

CVS Health is seeking a highly motivated and compassionate Healthcare Community Liaison to join our team. As a key member of our Accountable Care team, you will play a vital role in bridging the gap between our beneficiaries and the healthcare system.

Key Responsibilities
  • Provide outreach and education to beneficiaries, promoting health literacy and empowering them to take control of their health.
  • Collaborate with healthcare providers, nurse case managers, and other stakeholders to ensure seamless care coordination.
  • Identify and address socio-economic barriers to care, developing personalized health and social management plans for beneficiaries.
  • Conduct home visits and community outreach, building trust and rapport with beneficiaries from diverse cultural and socio-economic backgrounds.
  • Work closely with our team to develop and implement strategies to improve health outcomes, reduce unnecessary hospitalizations, and enhance overall well-being.
Requirements
  • 1+ year of experience working with patients/community members with clinical or behavioral health needs.
  • 1+ year of experience outreaching to community and helping to locate resources.
  • Willingness to travel 50-75% and work independently.
  • Strong interpersonal and communication skills, with the ability to work effectively with diverse constituencies.
  • Ability to work collaboratively in a team, manage multiple priorities, and exercise sound judgment.
Preferred Qualifications
  • Bilingual in Spanish and English.
  • Licensed Practical Nurse (LPN) or equivalent experience.
  • Community Health Worker Certification or equivalent training and experience.
  • 3-5 years relevant experience, with experience in serving Medicaid or Medicare populations in urban or rural environments.
