Job Summary

Thom Child and Family Services is seeking a highly skilled and compassionate Social Worker to join our team. As an Early Intervention Social Worker/Clinician, you will play a vital role in providing home and community-based early intervention services to infants and toddlers who have or are at risk for developmental delays and their families.

Key Responsibilities:
  • Conduct Multidisciplinary Assessments: Participate in team assessments of the client's developmental status to identify areas of strength and need.
  • Provide Direct Child Development Services: Offer developmentally appropriate services and activities for children 0-3 and their families, promoting healthy growth and development.
  • Develop Individualized Service Plans: Collaborate with families to create and implement Individual Family Service Plans (IFSPs), setting goals and objectives for the child's development.
  • Document Progress and Communication: Maintain accurate and detailed records of progress, assessments, and correspondence with families and other professionals.
  • Provide Intervention and Support: Deliver individual and group interventions for infants and toddlers with developmental delays or at risk for delays, focusing on cognitive, adaptive behavior, social-emotional, self-care, fine motor, gross motor, and communication development.
  • Act as a Service Coordinator: Schedule, develop, implement, monitor, and evaluate intervention plans for each family on caseload, ensuring timely and effective service delivery.
  • Support Families and Children: Offer professional assistance to families, facilitating parent-child interactions and empowering them to act as advocates for their child's needs.
Requirements:
  • Master's Degree in Social Work: Possess a Master's degree in Social Work from an accredited school of social work or be a certified or licensed social worker (LCSW, LICSW).
  • Experience with Infants and Toddlers: Have experience working with infants, toddlers, and families, preferably in a pediatric setting.
  • Knowledge of Family Systems and Early Childhood Development: Demonstrate a strong understanding of family systems and early childhood development principles.
  • Therapeutic Relationships: Possess the ability to engage in meaningful, therapeutic relationships with children, parents/caregivers, and community partners.
  • Cultural Humility and Individualized Services: Commit to delivering culturally humble and individualized services that prioritize family needs and values.
  • Excellent Communication and Organizational Skills: Exhibit excellent written and verbal communication and organizational skills, with the ability to maintain accurate records and documentation.
  • Confidentiality and Professional Boundaries: Adhere to confidentiality and professional boundaries standards, ensuring the highest level of integrity and respect for families and children.
  • Computer Skills and Documentation: Possess good computer skills and strong documentation skills, with the ability to maintain accurate and detailed records.
  • Driver's License and Reliable Transportation: Hold a current driver's license and have reliable transportation for work-related activities.
  • Bilingual/Bicultural a Plus: Possess bilingual/bicultural skills, which may be beneficial in serving diverse families and communities.
