About the Role

We are seeking a skilled Licensed Practical Nurse to join our team at Partners Healthcare System. As a Home Care LPN, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Key Responsibilities
  • Provide comprehensive nursing care to patients in their homes, including wound care, medication management, and vital sign monitoring.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop personalized care plans to meet those needs.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure seamless care coordination.
  • Communicate effectively with patients, families, and caregivers to ensure understanding of care plans and progress.
  • Document patient information accurately and thoroughly, maintaining confidentiality and adhering to HIPAA guidelines.
  • Participate in ongoing education and training to stay current on best practices and industry developments.
Requirements
  • Current LPN licensure in Massachusetts
  • Minimum 1 year of experience in acute care or sub-acute care settings
  • Home care experience preferred
  • Excellent written, verbal, and electronic communication skills
  • Ability to work independently and as part of an interdisciplinary team
  • Strong organizational skills and ability to adapt to unpredictable circumstances
  • Travel required in local geographic area
