Key Responsibilities- Train patients and their care partners in self-care dialysis modalities, including peritoneal dialysis and hemodialysis.
- Conduct home visits to assess the patient's home environment and evaluate their performance and home environment.
- Communicate patient assessment results to physicians and team members.
- Document all nursing services in the Electronic Medical Record.
- Participate in infection control monitoring and implementation.
- Ensure clinic and patient supply inventory and usage is appropriate.
- Continuously seek to improve patient and quality outcomes through tracking and trending of program and patient performance.
- Knowledge of federal, state, and local laws and regulations governing home dialysis programs and patient care requirements.
- Ability to work independently and as part of a team.
- Excellent communication and documentation skills.
- Ability to work flexible hours, including nights and weekends.
