Palliative Care Nurse Practitioner/Physician Assistant Primary Care Specialist

Eventus WholeHealth is seeking a highly skilled Palliative Care Nurse Practitioner/Physician Assistant to join our team. As a Palliative Care Nurse Practitioner/Physician Assistant, you will be responsible for delivering high-quality palliative care to patients with medical necessity for palliative service via in-person and telehealth options.
Key Responsibilities:- Deliver high-quality palliative care to patients with medical necessity for palliative service via in-person and telehealth options.
- Assist with the design of operational processes associated with the development of the palliative care program.
- Work with facilities and Eventus employees to ensure the growth of the palliative care program.
- Train and educate internal employees on palliative care principles and processes.
- Recognize gaps in processes and work with interdepartmental team members to streamline/organize palliative operations.
- Provide primary-care services in accordance with Eventus WholeHealth site-specific protocols and consistent with the standard of care for the specialty of family medicine, geriatric medicine, and/or internal medicine as it pertains to long-term care facility work.
- Masters degree in Nursing from an accredited educational institution and a current and valid license to practice medicine issued by the state in which you reside Medical Board.
- OR Have graduated from a physician assistant educational program that is accredited by a National Commission on Accreditation of Allied Education Programs.
- Have passed the national certification examination of the National Commission on Accreditation of Certification of Physician Assistants.
- Licensed by the state in which you reside as a Physician Assistant.
- Specialization in geriatric, family, or psychiatry desired.
