Job Summary:
As a Hospice Nurse, you will be responsible for managing the overall plan of care for a caseload of hospice patients in collaboration with other members of the interdisciplinary group (IDG). You will assess and identify patient/caregiver problems, establish nursing goals, and interventions. You will work closely with physicians to establish medical plans of care for patients and establish and maintain therapeutic relationships with patients and caregivers, including providing emotional support and teaching.
Responsibilities:
- Manage the overall plan of care for a caseload of hospice patients
- Assess and identify patient/caregiver problems, establish nursing goals, and interventions
- Work closely with physicians to establish medical plans of care for patients
- Establish and maintain therapeutic relationships with patients and caregivers
- Provide emotional support and teaching to patients and caregivers
- Document the outcome of care
- Provide dietary counseling for patient care needs as indicated
- Assume on-call responsibilities as scheduled
Requirements:
- Graduate of an accredited School of Nursing
- Currently licensed as an RN/LPN in North Carolina and in good standing with NCBON
- Experience in an acute, hospice, home health setting, or long-term care setting a plus
Other:
Valid state-issued driver's license required. Must carry automobile liability insurance at limits required by agency. Must have own transportation.
