Job Description
We are seeking a Registered Nurse to join our team as a Hospice RN/Care Manager. In this role, you will be responsible for providing direct patient care services, assessing patient needs, developing and implementing individualized care plans, and collaborating with an interdisciplinary team to ensure high-quality patient outcomes. You will also provide education and support to patients and their families, empowering them to make informed decisions about their care. As a Hospice RN/Care Manager, you will have the opportunity to make a meaningful difference in the lives of others while working in a dynamic and supportive environment.
Key Responsibilities
- Provide direct patient care services, including assessments, medication management, and wound care.
- Develop and implement individualized care plans that meet the unique needs of each patient.
- Collaborate with an interdisciplinary team, including physicians, therapists, and social workers, to ensure high-quality patient outcomes.
- Provide education and support to patients and their families, empowering them to make informed decisions about their care.
Required Skills and Qualifications
- RN license in the state of employment (or eligibility to obtain).
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology.
- Demonstrated ability to work independently and collaboratively as part of a team.
- Excellent communication and interpersonal skills.
- Ability to empathize with patients and their families, providing compassionate and supportive care.
