Job Description
This Full-Time Hospice RN/RN Case Manager role offers a unique opportunity to provide high-quality patient care and support to individuals facing serious illness or terminal diagnosis. As a key member of our interdisciplinary team, you will be responsible for coordinating patient care services, collaborating with healthcare professionals, and empowering patients and their families to navigate the end-of-life journey with dignity and comfort.
Salary and Benefits
The estimated annual salary for this position is $85,000-$110,000, depending on experience and location. Additionally, Gentiva Hospice offers a comprehensive benefits package, including medical, dental, vision plans, wellness program, and generous paid time off. Required Skills and Qualifications
- Hospice heart: compassionate, kind, and empathetic to patients and their families.
- Comfortable providing care wherever the patient calls "home".
- Demonstrate flexibility and composure while supporting others in a time of crisis.
- RN license in the state of requested employment (or eligibility to obtain).
- 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred.
