Private Duty Nurse LVN-Full Time/Part Time Days

Job SummaryWe are seeking a skilled and compassionate Licensed Practical Nurse (LPN/LVN) to provide one-on-one care to our patients in the comfort of their own homes. As a Private Duty Nurse, you will work closely with our patients, their families, and our healthcare team to ensure the highest level of care and support.
Responsibilities- Implement and coordinate care plans in accordance with physician orders
- Provide skilled nursing care, including wound care, medication management, and vital sign monitoring
- Document all patient interactions, assessments, and care provided in a timely and accurate manner
- Collaborate with the healthcare team to ensure seamless care transitions and communication
- Provide education and support to patients and their families on disease management, self-care techniques, and prevention strategies
- Current, unrestricted state license as a Licensed Nurse in the state of practice
- Current CPR certification
- One year of experience working as a Licensed Nurse in a clinical care setting
- Ability to work independently and as part of a team
- Strong communication and interpersonal skills
