About the Role

We are seeking a skilled Physical Therapist Assistant to join our innovative Delta Health Rehab at Home program. As a PTA, you will play a vital role in delivering high-quality outpatient physical therapy to patients in the comfort of their own homes.

Key Responsibilities
  • Administer physical therapy to patients under the supervision of a Physical Therapist
  • Develop and implement individualized treatment plans to improve or regain mobility and independence
  • Collaborate with healthcare providers to ensure coordinated and integrated services delivery
  • Provide education to patients and their families on in-home exercise programs, activities of daily living, and safe mobility practices
  • Participate in performance improvement activities and attend patient care conferences as scheduled
Requirements
  • Graduate from a two-year college level program approved by the American Physical Therapy Association
  • Currently licensed in the state(s) in which practicing
  • Three (3) years' experience, preferred
  • Background in Home Health and Outpatient settings, preferred
