About the Role

We are seeking a skilled and compassionate Home Health Nurse to join our team at CenterWell Home Health. As a Home Health Nurse, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Key Responsibilities
  • Provide professional nursing services under the direction of a RN in compliance with state and federal regulations, applicable healthcare standards, and CenterWell Home Health policies and procedures.
  • Develop and implement individualized care plans to meet the unique needs of each patient, focusing on rehabilitation, recovery, and independence.
  • Conduct thorough assessments, identify patient needs, and collaborate with healthcare teams to ensure comprehensive care.
  • Communicate effectively with patients, families, and healthcare professionals to ensure seamless care coordination and transitions.
  • Maintain accurate and detailed records of patient interactions, progress, and outcomes.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements
  • Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Valid drivers license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years of experience as an LPN/LVN in a clinical setting, preferably in home health or hospice care.
