Clinical Nurse Practitioner or Physician Assistant

Matrix Medical Network is seeking a skilled Nurse Practitioner or Physician Assistant to join our team in providing high-quality care to at-risk individuals in their homes. As a member of our team, you will conduct comprehensive health assessments, review medical histories, and provide education and support to patients and their families.
Key Responsibilities:
- Conduct adult and geriatric assessments, medical history, diagnosis, and treatment
- Work collaboratively with physicians, case managers, social workers, and other healthcare professionals
- Provide education and support to patients and their families
- Collaborate with primary care physicians on patient education and follow-up
Requirements:
- Master's degree in nursing or a related field
- Current RN and NP or PA licensure in the state of practice
- Board certification in a Matrix-approved specialty
- 1 year of experience as a Nurse Practitioner or Physician Assistant
- Strong computer skills and familiarity with EMR software
- Excellent verbal and written communication skills
Travel Requirements:
- Ability to travel to meet patients in their homes
- Valid state driver's license and adequate automobile insurance coverage
