We are seeking a dedicated and compassionate Patient Advocate Specialist to join our team in East Houston.

About the Role

This is a unique opportunity to make a positive impact on people's lives by providing support, guidance, and assistance to clients in need. As a Patient Advocate Specialist, you will be responsible for assessing client needs, developing personalized service plans, and coordinating care with various healthcare providers.

Key Responsibilities
  • Critical Assessments: Conduct comprehensive assessments to identify client needs, strengths, and goals.
  • Personalized Support: Provide emotional support and guidance to clients through regular check-ins and meetings.
  • Advocacy and Liaison: Advocate for clients and act as a liaison between them and healthcare providers.
  • Care Coordination: Monitor client progress, reassess needs, and adjust case plans as needed.
  • Crisis Intervention: Provide crisis intervention when necessary and offer referrals to appropriate resources.
  • Team Collaboration: Work collaboratively with other professionals, including social workers, healthcare providers, and community organizations.
  • Confidentiality: Ensure all client information is handled with confidentiality and respect.
Qualifications

To be successful in this role, you will need:

  • A Bachelor's degree in Social Work, Psychology, Nursing, or a related field (A Master's degree may be preferred).
  • Proven experience in case management, social work, or a related field.
  • Strong understanding of social services, community resources, and relevant regulations.
  • Excellent communication and interpersonal skills, with the ability to establish trust and rapport with clients.
  • Ability to work independently and as part of a team in a fast-paced environment.
  • Acute Care hospital experience.
  • Bilingual (Spanish required).
Compensation

The salary for this role is $60,000 per year, based on experience and negotiation. We offer a competitive benefits package, including:

Schedule

This is a full-time position, working 3 12-hour shifts per week.
