Job Summary

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team at Amedisys. As a Hospice Nurse, you will provide patient-centered care to individuals in need of end-of-life support, working collaboratively with our interdisciplinary team to ensure the best possible outcomes.

Responsibilities
  • Perform initial nursing assessments on new admissions and develop individualized care plans
  • Collaborate with the care team to ensure continuity of patient care and make referrals to other disciplines as needed
  • Communicate patient progress to the clinical manager, physician, and care team
  • Develop, prepare, and maintain accurate and timely patient care progress records
  • Provide education to families on patient condition, needs, and care
  • Perform on-call responsibilities and services to patients/families as assigned
Requirements
  • Current RN license, specific to the state(s) you are assigned to work
  • One+ year of nursing experience required
  • Current CPR certification
  • Valid driver's license, reliable transportation, and liability insurance
