Job Summary

We are seeking a skilled and compassionate Hospice LPN to join our team at Sanford Hospice Centennial Cottage. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals in our Hospice Residence and patient homes.

Responsibilities
  • Assume a lead role with the interdisciplinary team, working collaboratively with LPNs, Social Workers, CNAs, Spiritual Advisors, and MDs to deliver comprehensive care.
  • Provide symptom and pain control therapeutic treatment, completing admission assessments and care-planning across physical and psychosocial domains.
  • Offer client care, including bathing, dressing, feeding, and physical therapy as directed, utilizing critical thinking skills to recognize changes in condition or mentation.
  • Liaise with Physicians/Nurse Practitioners and other healthcare workers to support clients and families.
  • Administer medications as prescribed by physicians and maintain accurate records.
Requirements
  • Graduate of an accredited school of nursing with a current license in good standing.
  • At least one year of experience in hospice care, with AHA BLS certification.
