Hospice Nurse

We are seeking a compassionate and skilled Hospice LPN to join our team at Sanford Hospice Centennial Cottage. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals in our Hospice Residence and patient homes.
Responsibilities- Assume a lead role with the interdisciplinary team to provide holistic, evidence-based care to patients and their families.
- Collaborate with the physician, nurse practitioner, and other healthcare workers to support patient care and family needs.
- Provide symptom and pain control therapeutic treatment, complete admission assessments, and care-planning across physical and psychosocial domains.
- Deliver client care, including bathing, dressing, feeding, and physical therapy as directed.
- Liaise with the physician and other healthcare workers to support patient care and family needs.
- Graduate of an accredited school of nursing with a current license in good standing.
- One year of experience in hospice care, with a focus on end-of-life care and symptom management.
- AHA BLS certification required.
