Adult Home Health Nurse

Job Summary: We are seeking a skilled and compassionate Adult Home Health Nurse to join our team. As a member of our community health team, you will provide high-quality nursing care to patients in their homes, promoting independence and improving their overall well-being.

Responsibilities:

  • Provide skilled nursing care to patients in their homes, including wound care, medication management, and education on self-care.
  • Conduct thorough assessments of patients' physical and emotional needs, developing individualized care plans to address their unique needs.
  • Collaborate with other healthcare professionals, including physicians, therapists, and social workers, to ensure comprehensive care.
  • Communicate effectively with patients, families, and caregivers to ensure seamless care transitions and address any concerns.
  • Document patient progress, including assessments, care plans, and outcomes, in accordance with regulatory requirements.

Requirements:

  • Greater than two years of home or public health nursing experience preferred.
  • Previous home care experience desired but not required; we are willing to train and develop a candidate who demonstrates a willingness and desire to learn home health nursing.
  • Graduate nurses are considered for our Community Health Nurse I level.

Benefits:

  • $10,000 Sign On Bonus for full-time external applicants, prorated for part-time roles.
  • Mileage reimbursed at Federal Gas Rates.

Work Environment:

  • Our coverage area serves the Chittenden and Grand Isle Counties and is segmented into four territories.
  • Exact case load territory is assigned after orientation period and may not be the same as listed on this posting.

On-Call: Not Required.
