About the Role:

The University of Vermont Health Network is seeking a skilled Registered Nurse to join our Adult Home Care team. As a Community Health Nurse, you will deliver high-quality care to patients in their homes, providing a wide range of skilled nursing services while working to the top of your licensure.

Key Responsibilities:

  • Provide comprehensive nursing care to patients in their homes, including wound care, medication management, and education.
  • Collaborate with a multidisciplinary team to develop and implement patient care plans.
  • Conduct thorough assessments of patients' physical and emotional needs, identifying opportunities for improvement and developing strategies to address them.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions.
  • Participate in ongoing education and professional development to stay current with best practices and industry advancements.

Requirements:

  • Greater than two years of home or public health nursing experience preferred.
  • Previous home care experience desired but not required; we are willing to train and develop a candidate who demonstrates a willingness and desire to learn home health nursing.
  • Graduate nurses are considered for our Community Health Nurse I level.

  • $10,000 Sign On Bonus for external applicants, prorated for part-time roles.
  • Additional terms and conditions apply.
  • Mileage reimbursed at Federal Gas Rates.
