Salary: $65,000 - $85,000 per year, depending on experience and location.

As a Care Case Manager - LPN, you'll have access to opportunities for professional development, ongoing programs for your overall health and wellness, and a full suite of health insurance, life insurance, and retirement plans.
Responsibilities:
- Coordinate Care: Serve as a liaison between Brookdale teams, residents, families, and healthcare providers to ensure seamless care coordination.
- Review Medical Diagnoses: Analyze the current population's medical diagnoses to identify care needs and assess the health, functional, and psycho-social status of residents.
- Collaborate with Healthcare Providers: Work closely with primary and specialty care providers to formulate resident-specific care plans and ensure the delivery of medically necessary services aligns with established care plans.
- Participate in Collaborative Care Reviews: Participate in collaborative care reviews (CCR) and care conferences to ensure high-quality care delivery.
- Communicate with Residents and Families: Educate and explain Brookdale's care pathways, provide advanced care planning education, and ensure each resident has a completed Advanced Care Plan, inclusive of goals of care.
- Apply Quality Improvement Methodology: Analyze, enhance, and manage outcomes to achieve quality measure goals, while collaborating with HWD and/or RN Care Manager to ensure compliance with service plans.
- Document and Communicate: Complete documentation as required by Brookdale or state regulatory agencies and communicate information promptly to the appropriate care provider regarding changes in residents' health/well-being.
Qualifications:
- LPN License: Graduate from an accredited LPN program and has successfully passed the state licensing test.
- Knowledge of Value-Based Care Models: Knowledge of value-based care models is preferred.
- Experience in SNF and HH Environments: Experience working in SNF and HH environments is a preferred.
- Certifications, Licenses, and Other Special Requirements: LPN license required.
