Position Overview:

We are in search of a dedicated and skilled homecare manager to become a vital part of our leadership team at Assisting Hands Serving Carlsbad and Oceanside. The Care Manager will collaborate closely with our ownership, playing a crucial role in managing the provision of care solutions and services to our clients. The ideal candidate will be tasked with formulating care plans, evaluating client needs, supervising caregivers, and acting as a bridge between clients, caregivers, and other healthcare professionals. Additionally, the Care Manager will engage in various facets of the Company's operations, including business growth, sales, and marketing.

Responsibilities:

1. Conduct thorough assessments of client needs and develop personalized care plans in collaboration with clients, their families, and other healthcare professionals.

2. Organize and schedule care services based on client preferences, caregiver availability, and specific care requirements.

3. Recruit, train, supervise, and evaluate caregivers to ensure they provide exceptional care in line with company policies and standards.

4. Perform regular home visits to evaluate client satisfaction, monitor care plan effectiveness, and address any concerns that may arise.

5. Act as a primary contact for clients and their families, offering support, guidance, and advocacy as necessary.

6. Collaborate with healthcare providers, including physicians, nurses, therapists, and social workers, to coordinate and effectively integrate care services.

7. Maintain accurate and current documentation of client assessments, care plans, and service delivery in compliance with regulatory requirements and company standards.

Qualifications:

1. Minimum of 3 years' experience as an LVN and/or as a seasoned homecare manager.

2. Strong problem-solving skills with a proactive attitude.

3. Excellent interpersonal, communication, and leadership abilities, with a talent for effective collaboration with clients, caregivers, and office team members.

4. Proficient in computer skills, including Microsoft Office applications. Experience with care management and scheduling software is preferred.

5. Compassionate, empathetic, and client-focused approach to care management.

6. Valid driver's license and dependable transportation.

Compensation: $45.00 per hour
