Job Title: Program Assistant - Community Support

At BrightPath LLC, we are seeking a compassionate and dedicated Program Assistant to join our team. As a Program Assistant, you will play a vital role in supporting individuals with disabilities and mental health challenges in their communities.

Key Responsibilities:
  • Provide direct care and support to individuals living independently, helping them acquire essential adaptive skills.
  • Assist individuals in developing personalized 24-Hour Emergency Back-Up Plans.
  • Support the Designated Coordinator with clerical tasks, ensuring efficient service delivery.
  • Document progress, report incidents, and maintain compliance with 245D regulatory guidelines.
  • Collaborate with a wide range of professionals, including waivered service providers, mental health teams, and social workers.
  • Participate in an on-call rotation for 24-Hour Emergency Assistance services.
Requirements:
  • High school diploma or GED required, with at least 1 year of experience in a 245D-regulated field.
  • Compassionate, empathetic, and patient, with strong problem-solving skills.
  • Strong organizational and time-management skills.
  • Valid driver's license and access to a reliable, insured vehicle (mileage reimbursement provided)
  • Must pass criminal/registry background checks in accordance with Minnesota DHS guidelines.
